Step 1 – Visit to the PMJAY official website (https://pmjay.gov.in/).

Step 2 – Click on the PMJAY ‘Am I Eligible’ option right corner near the search bar.

Step 3 – Then you have to Enter your name, phone number, ration card number or the Rashtriya Swasthya Bima Yojana URN number.

Step 4 – On submitting your mobile number, you’ll receive an OTP.
Step 5 – Submit the generated OTP.

Step 5 – Then Select your state of residence.

Step 6 – Click on the ‘Search’ button once you’ve filled in all the relevant details. 

Step 7 – Now you will see a list of names that will fall under the category.

Step 2: Then you have to Log in with your registered phone number.

Step 3: Input the captcha code and generate the OTP.

Step 5: Provide the accurate HHD code to the PMJAY Common Service Centre (CSC).
